Use this form to request access to Animal Surgery Operating Room (ASOR) managed facilities.

Prior to completing the form, please note the following:

• The person requesting card reader access must have a valid University of Michigan MCard (identification card)
• Completion of this form does NOT provide additional building and/or animal housing access
• Orientation is required prior to access being granted
