All other documents can be maintained in the lab and must be available for review within 4 hours of request by veterinary or regulatory personnel (e.g. IACUC, AAALAC).\r\n<ol start=\"1\">\r\n \t<li>Rodents and non-mammals: Surgical and post-operative records for survival surgeries<\/li>\r\n \t<li>Non-rodent mammals: Treatment cards, surgical and post-operative records (including anesthetic monitoring for all surgeries, whether survival or terminal), disposition records for dogs and cats<\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li>Non-USDA-regulated species\r\n<ol start=\"1\">\r\n \t<li>Investigative personnel place records (or copies of records) within one week of final record entry into the drop box in the animal housing rooms for animals not covered by the <a href=\"https:\/\/az.research.umich.edu\/animalcare\/informational\/usda-animal-welfare-act-and-animal-welfare-regulations-2017\/\"><em>Animal Welfare Act <\/em>including:<\/a>\r\n<ol style=\"list-style-type: lower-alpha\" start=\"1\">\r\n \t<li>Mice (genus Mus)<\/li>\r\n \t<li>Rats (genus Rattus)<\/li>\r\n \t<li>Fish<\/li>\r\n \t<li>Reptiles<\/li>\r\n \t<li>Amphibians<\/li>\r\n \t<li>Captive-bred birds<\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li>Husbandry personnel collect the records once per calendar month and turn into the ULAM husbandry supervisor.\r\n<ol style=\"list-style-type: lower-alpha\" start=\"1\">\r\n \t<li>Husbandry supervisor submits to the ULAM business office for veterinary review and storage.<\/li>\r\n<\/ol>\r\n<\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li>USDA-regulated species\r\n<ol start=\"1\">\r\n \t<li>Investigative personnel submit records (or copies of records) within one week of the final entry in the record to ULAM for all animals covered by the Animal Welfare Act.\r\n<ol style=\"list-style-type: lower-alpha\" start=\"1\">\r\n \t<li>ULAM stores the records long-term according to the <a href=\"https:\/\/az.research.umich.edu\/animalcare\/sops\/procedures-veterinary-medical-records\/\">Procedures for Veterinary Medical Records<\/a><\/li>\r\n \t<li>Examples of final entries are:\r\n<ol style=\"list-style-type: lower-roman\" start=\"1\">\r\n \t<li><a class=\"gtip\" href=\"https:\/\/az.research.umich.edu\/animalcare\/glossary\/euthanasia\/\">Euthanasia <\/a><\/li>\r\n \t<li>The end of post-surgical monitoring<\/li>\r\n \t<li>The resolution of a <a class=\"gtip\" href=\"https:\/\/az.research.umich.edu\/animalcare\/glossary\/clinical-case\/\">clinical case <\/a><\/li>\r\n \t<li>The transfer to another institution or laboratory<\/li>\r\n<\/ol>\r\n<\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li>Confirm the records contain the following information before submitting to ULAM (documents without the required information are not accepted):\r\n<ol style=\"list-style-type: lower-alpha\" start=\"1\">\r\n \t<li>Protocol number<\/li>\r\n \t<li><a class=\"gtip\" href=\"https:\/\/az.research.umich.edu\/animalcare\/glossary\/principal-investigator-pi\/\">Principal investigator<\/a> last name<\/li>\r\n \t<li>Animal's <a class=\"gtip\" href=\"https:\/\/az.research.umich.edu\/animalcare\/glossary\/clinical-number\/\">clinical number<\/a> (if one has been assigned by ULAM personnel)<\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li>Place completed records in the <a class=\"gtip\" href=\"https:\/\/az.research.umich.edu\/animalcare\/glossary\/animal-treatment-report-atr\/\">ATR<\/a> \/Records drop-boxes. These boxes are located:\r\n<ol style=\"list-style-type: lower-alpha\" start=\"1\">\r\n \t<li>Outside the veterinary technicians' offices in MSII, BSRB, CH, Brehm, and NCRC.<\/li>\r\n \t<li>Outside break rooms in MSRB and BSRB.<\/li>\r\n \t<li>Contact the husbandry supervisor in your area for ATR drop-box locations in other buildings.<\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li>It may be acceptable to submit documents to the ULAM veterinary team electronically if discussed and approved by the faculty veterinarian for the area.<\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li>Disposition of dogs and cats:\r\n<ol start=\"1\">\r\n \t<li>Generate <a class=\"gtip\" href=\"https:\/\/az.research.umich.edu\/animalcare\/glossary\/record-of-disposition\/\">Records of disposition<\/a> for all dogs and cats using USDA APHIS Form 7006 (see Appendix G) and include:\r\n<ol style=\"list-style-type: lower-alpha\" start=\"1\">\r\n \t<li>USDA identification number.<\/li>\r\n \t<li>Description of animal (species, breed, markings, sex, age).<\/li>\r\n \t<li>Date and manner of disposition (euthanasia, transfer, etc.).<\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li>Investigative personnel turn in the disposition records to the veterinary technicians.\r\n<ol style=\"list-style-type: lower-alpha\" start=\"1\">\r\n \t<li>The veterinary technicians store as part of the animal's medical record according to the Procedures for Veterinary Medical Records.<\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li>Promptly return USDA-numbered collars\/tags from euthanized dogs and cats to the ULAM Husbandry Supervisors.<\/li>\r\n<\/ol>\r\n<\/li>\r\n<\/ol>\r\n<\/li>\r\n<\/ol>","legacy_media_position":"","legacy_media_file":"","legacy_media_url":"","legacy_glossary_term":"","legacy_glossary_nids":"","legacy_resource":"","legacy_resource_nids":"","legacy_buttons":null},{"legacy_section_type":"heading","legacy_heading":"Appendices","legacy_subheading":"3. These animals are not covered by the <a href=\"\/animalcare\/informational\/usda-animal-welfare-act-and-animal-welfare-regulations-2017\"><em>Animal Welfare Act<\/em><\/a>.\r\n<ol style=\"list-style-type: decimal\" start=\"1\">\r\n \t<li>Once the surgical monitoring period is complete, investigative personnel clip records (or copies thereof) and place them into the drop box in the animal room. These records will be collected by husbandry personnel once per calendar month.<\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li>All other mammals (covered by the Animal Welfare Act):\r\n<ol style=\"list-style-type: decimal\" start=\"1\">\r\n \t<li>Records\u00a0<strong>must be submitted to ULAM<\/strong>\u00a0for long-term storage within one week of the final entry in the record (usually <a class=\"gtip\" href=\"\/animalcare\/glossary\/euthanasia\">euthanasia<\/a>, the end of post-surgical monitoring, the resolution of a <a class=\"gtip\" href=\"\/animalcare\/glossary\/clinical-case\">clinical case<\/a>, or transfer to another institution or laboratory).\r\n<ol style=\"list-style-type: lower-alpha\" start=\"1\">\r\n \t<li>It is highly recommended to submit a\u00a0<strong>copy<\/strong>\u00a0of these documents.<\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li>Confirm the record contains the following information before submitting to ULAM (Documents without this information will not be accepted):\r\n<ol style=\"list-style-type: lower-alpha\" start=\"1\">\r\n \t<li>Protocol number<\/li>\r\n \t<li><a class=\"gtip\" href=\"\/animalcare\/glossary\/principal-investigator-pi\">Principal investigator<\/a> last name<\/li>\r\n \t<li>Animal's <a class=\"gtip\" href=\"\/animalcare\/glossary\/clinical-number\">clinical number<\/a> (if one has been assigned by ULAM personnel)<\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li>Deposit completed records in the <a class=\"gtip\" href=\"\/animalcare\/glossary\/animal-treatment-report-atr\">ATR<\/a>\/Records drop-boxes.
