For Michigan Medicine systems, you may need to enter it in this format: uniqname@med.umich.edu<\/span><\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">UMICH Password - Your UMICH password is the password for your University of Michigan account.<\/span><\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">1Kosmos - These automated emails provide information about your identity verification status and U-M\u00a0 login credentials.<\/span><\/li>\r\n<\/ul>\r\n<\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">1Kosmos will send a text message with a link to upload documents. <\/span><\/li>\r\n<\/ul>\r\n&nbsp;","legacy_media_position":"","legacy_media_file":"","legacy_media_url":"","legacy_glossary_term":"","legacy_glossary_nids":"","legacy_resource":"","legacy_resource_nids":"","legacy_buttons":null},{"legacy_section_type":"heading","legacy_heading":"Procedure","legacy_subheading":"","legacy_section_text":"","legacy_media_position":"","legacy_media_file":"","legacy_media_url":"","legacy_glossary_term":"","legacy_glossary_nids":"","legacy_resource":"","legacy_resource_nids":"","legacy_buttons":null},{"legacy_section_type":"text_area_with_subheading","legacy_heading":"","legacy_subheading":"Step 1: Verify Your Identity","legacy_section_text":"<span style=\"font-weight: 400\">All vendors and contractors are required to complete identity verification before receiving access to academic research applications used at Michigan Medicine\u00a0\u00a0<\/span>\r\n\r\n<strong>Identity Verification Overview<\/strong>\r\n<ol>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">The CTMS team will send a short, secure survey to collect your contact information.<\/span><\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">The Michigan Medicine Identity Management team will send an email inviting you to start the identity verification process.<\/span><\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">You will enroll in the 1Kosmos identity verification system.<\/span><\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">You will submit the required documents to confirm your identity.<\/span><\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">1Kosmos will review your documentation<\/span><\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Once your identity is verified, you will provide 1Kosmos with consent to share with Michigan Medicine only the information necessary to create or renew your U-M login credentials.<\/span>\r\n<ol>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">If 1Kosmos cannot provide verification, an exception request may be submitted on your behalf.<\/span><\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">If the exception request is approved, your U-M login credentials will be created or renewed.<\/span><\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">You will be notified if U-M login credentials cannot be created or renewed.<\/span><\/li>\r\n<\/ol>\r\n<\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">The Michigan Medicine Identity Management team will send an email containing your U-M login credentials within 1-3 business days.<\/span><\/li>\r\n<\/ol>\r\n<strong>Helpful Information - 1Kosmos<\/strong>\r\n<ul>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Use your computer, not your smartphone or the 1Kosmos mobile app, to complete all 1Kosmos steps\u00a0<\/span><\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Use the same contact information you provided on the initial survey to log in to 1Kosmos:<\/span>\r\n<ul>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Username: email address\u00a0<\/span><\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Password: phone number<\/span>\r\n<ul>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">US-based phone number: include dashes<\/span><\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">International-based phone number: do not include dashes<\/span><\/li>\r\n<\/ul>\r\n<\/li>\r\n<\/ul>\r\n<\/li>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">The LiveID identification pathway includes a comparison of a selfie with the photo on an identity document<\/span>\r\n<ul>\r\n \t<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Once logged into 1Kosmos, on the profile page, click Login Options, then click on the Setup down arrow on the right-hand side of the screen.
